Patients are typically entitled to: Quality care in a hospital with no discrimination or bias. An environment that is safe, clean and free of neglect or abuse. Be informed and involved in all health decisions and care. Have their privacy protected and personal information kept confidential. Be prepared when they leave the hospital with a continued care plan.  

Most Common Patient Safety Issues:

Health Care Acquired Infections: Such as a superbug, sepsis, or from the reuse of improperly disinfected medical tools. Diagnosis Errors: Failure to diagnose, misdiagnosis or delay in diagnosis. Vulnerability of Computerized Medical Devices: Devices can be hacked, giving the wrong dosage of medication. Medication Errors: Wrong combinations, wrong dosage, or wrong medication. Home or Post-Acute Care Following Discharge: Hospital failure to create and follow up with a plan so that a patient does not require re-admittance.   Become a proactive partner in your healthcare. Communication is key. Be empowered. Ask healthcare providers to:
  1. Double check medications;
  2. Wash hands appropriately and take required precautions to reduce spread of infections;
  3. Clarify their diagnosis and get second opinions; and
  4. Take necessary care and precautions to prevent falls.
While in the hospital, what to do if you see an error:
  1. Do not wait to report an incident.
